Registration is now open for the 2021-2022 school year for new students! If you have an incoming K-8 child(ren) new to the Evesham Township School District (ETSD), now is the best time to register. Parents are encouraged to visit the district website at www.evesham.k12.nj.us and click on the registration link under “Announcements.”
For all families with an incoming kindergarten and/or first-grade child, separate Google Form links are posted by each elementary school. Simply select the form for your child’s anticipated home school. For any new student entering grades 2-8, there is a direct link to a district form where anticipated school and grade level can be designated in the first question. If you are unsure of the school your child will be attending, see the school zone
and neighborhood list below. This information is also posted on the website.
Submission of the Google Form initiates the registration process. The school office will contact those who submit the form to set up an individual registration appointment at the school to verify documentation and finalize registration for the new school year.
In order for a child to be eligible for kindergarten, he/she must be five years old on or before October 1st of the school year. For first grade, children must be six years old on or before October 1st of the school year. For parents with children entering grade one-eight, previous schooling information/records will also be collected at the time of registration. All parents are requested to provide documentation (listed below) and the completed forms at the registration appointment. Registration packets with forms are available on the website.
The following documents must be submitted for admission to the Evesham Township School District:
● Four proofs of residence;
● Student’s most recent report card, transcript, and withdrawal form from the previous school, if not
already forwarded by the previous school;
● Proof of the following immunizations, in accordance with New Jersey State Department of Health, signed by a physician:
o Diphtheria, Whooping Cough and Tetanus (D.P.T.) – a total of four doses, one dose of which must
have been given on or after the child’s fourth birthday, or any combination of five doses.
o Polio Vaccine – three doses, with a third dose given on or after the child’s fourth birthday; or
any combination of four doses.
o Measles, Mumps, and Rubella (M.M.R.) – two doses, both immunizations must be after the first
birthday.
o Hepatitis B – three doses, given at mandated intervals.
o Varicella – one dose on or after the first birthday or proof of disease immunity. ● Current physical form, physicals are required for all children entering a New Jersey school for the first time (the exam must have been performed within 365 days of the first day of school for the
child);
● Student’s original or certified copy of the birth certificate (with parents’ name); and
● An optional dental examination form is recommended before entering school.
● If your child has an IEP, bring a copy of the current document, if not already forwarded by the previous school.
● Custody agreement, if applicable.
All documentation must be provided at the time of registration. A copy of your documents will be made and the originals returned to you.
